EVERY PLAYER MUST SUBMIT THEIR OWN INDIVIDUAL WAIVER.
*IF YOU REQUIRE A SUB FOR YOUR TEAM, YOUR SUB MUST ALSO FILL OUT A WAIVER. It is your responsibility to make sure they fill it out. Please make sure they do so prior to arriving in case they also need a parent signature.
If you (or your sub) has not filled out the waiver by our first night, June 24th, you will not be playing.

*There is also a separate form for "Free Agent" players who are signing up but are not committed to a team yet. The League Director can help connect you with a level-appropriate team.
Reminder: Payment is per TEAM, not individual!
